A Brixham, UK based fishing vessel operator has been ordered by a court to pay more than £100,000 after a vessel modification caused it to capsize, leading to the deaths of two people onboard. Joanna C had a major refit in 2019, including the addition of a whaleback, extension of the wheelhouse and raised bulwarks being added. That fatal incident happened in 2020 when the vessel’s gear snagged on the seabed. Its lack of stability meant it could not recover, causing the boat to sink rapidly. MAIB issue safety warning following fatal injury to recreational diver after contact diving support boat Karin
On the morning of 28 September 2023, a recreational diver carrying out decompression stops died, almost certainly as a result of being struck by the rotating propeller of the UK registered diving support boat Karin. The diver had been diving from a second dive boat that was also supporting divers exploring the wreck of the German battleship SMS Markgraf, which was lying at a depth of 45m in Scapa Flow, Orkney Islands, Scotland.

MAIB Report: Powerboat Awesome accident resulting in two fatalities
The MAIB has issued a report on its investigation of the powerboat Awesome accident near Little Thatch Island, British Virgin Islands on 2 October 2022, where the loss of control resulted in two fatalities.
On 2 October 2022, the mono-hulled offshore powerboat Awesome was returning to its home port when it unexpectedly veered to starboard, then rolled violently to port, ejecting 10 occupants. Awesome ended up in a vertical position, with its bow in the air, briefly staying afloat before it started to sink by the stern. Two of the occupants did not survive the accident. UK MAIB Safety Digest 2024 published
The Marine Accident Investigation Branch has published its the MAIB Safety Digest 2024 spring edition. It features a varied collection of lessons learned from recent marine accidents involving vessels from the merchant, fishing and recreational sectors.
MAIB Safety Digest 2024 draws the attention of the marine community to some of the lessons arising from investigations into recent accidents and incidents. It contains information that has been determined up to the time of issue.

MAIB report into the fatal Eder Sands man overboard incident
On 7 October 2022, a deckhand from the UK registered gill netter Eder Sands fell overboard while the vessel was shooting a net approximately 150 nautical miles west of Ireland. Despite an extensive 21-hour search that involved other vessels and fixed-wing aircraft, the crew member was not found.
Safety issues

Safety warning by MAIB following a fatal chain failure on scallop dredger Honeybourne III
On the night of 6 October 2023, a chain link in the lifting arrangement for the dredging gear that was suspended from the raised port derrick on the UK registered scallop dredger Honeybourne III (PD905) failed. The gear fell without warning and struck a deckhand working on the deck below; the deckhand suffered serious head injuries and died on board.

Capsize and sinking of fishing vessel Crig-A-Tana
The is a completed preliminary assessment (PA) summary 1/24: Crig-A-Tana by the MAIB.
At 1207 on 12 November 2022, the UK registered 7.47m potter Crig-A-Tana rapidly capsized and foundered approximately 6 nautical miles (nm) south-east of Lizard Point, Cornwall, England while engaged in fishing operations. The two crew members were uninjured and there was no significant pollution as a result of the accident.

Accidental discharge of condensed aerosol fire-extinguishing system on beam trawler Resurgam report issued
On 15 November 2019, an apprentice engineer died when a FirePro condensed aerosol fire-extinguishing system was inadvertently activated in the engine room of the fishing vessel Resurgam (PZ1001) as it was being installed. The apprentice engineer together with a shore engineer and two installation technicians were working in the engine room when the system activated, filling the engine room with the fire-extinguishing aerosol.

RRS Sir David Attenborough lifeboat davit failure report issued
The MAIB has issued an accident investigation report into the lifeboat davit failure aboard RRS Sir David Attenborough. On the 4 March 2021, the port lifeboat from RRS Sir David Attenborough fell into the water while the crew were practising lifeboat drills. The investigation found that a critical interlock device on the lifeboat davits had heavily corroded due to lack of maintenance. The interlock failed to operate correctly and subsequently caused the lifeboat to fall from the davit and into the sea with three crew on board. It also found that the installation of the lifeboat davits was not in accordance with the manufacturer’s instructions and did not meet international regulations. Collision between a general cargo vessel and a split hopper investigation report
The UK Marine Accident Investigation Branch (MAIB) has published its investigation report into the collision between the general cargo vessel Scot Carrier and the split hopper barge Karin Høj.
The collision resulted in the capsize of the barge with two fatalities in the Bornholmsgat traffic separation scheme. On 13 December 2021, the UK registered general cargo ship Scot Carrier and the Denmark registered split hopper barge Karin Høj collided in the precautionary area adjacent to the Bornholmsgat traffic separation scheme, Sweden. UK MAIB Annual Report 2022 published
UK Marine Accident Investigation Branch (MAIB) published its annual report for 2022, reporting that the branch received 1,263 reports of marine casualties and marine incidents involving UK vessels worldwide or other nations’ vessels in UK waters and commenced 16 investigations.

MAIB report published into the death of one person overboard from stern trawler Copious
At about 0300 on 18 February 2021, a deckhand fell overboard from the twin rig stern trawler Copious (LK 985) approximately 30 nautical miles south-east of the Shetland Islands. The deckhand was conscious, wearing a lifejacket and was quickly brought alongside the vessel. However, the crew’s attempts to recover the casualty back on board were unsuccessful. He was unresponsive when recovered from the water by a coastguard helicopter and pronounced dead on arrival at hospital.
Safety Issues
